Incision under breast
A very difficult case to get a pleasing result. The patient had
a wide chest. Her breast tissue was very dense and only occupies
a relatively small area on the chest. The distance between the
nipple and the fold under the breast is very short and, because
of the density of her tissue, the fold is very tight. She is at
high risk of developing a double fold where the original fold
can be seen above the new one that is formed by the bottom of
the implant. It was therefore important not to choose a wide implant
so that the bottom of the implant (which defines the new fold)
was not too far below the original breast fold. A very high profile
implant allowed the patient to get the volume she desired without
having to go too wide. The dual plane technique, where only the
top of the implant ends up under the muscle, allowed the implant
to expand the lower half of the breast stretching out the old
crease. This technique also ensured the top of the implant did
not look too round since it was covered by muscle. See patient
number 8 for a more about the dual plane technique.
